Referrals Checklist

To ensure a valid referral under Better Access, a letter from your GP, psychiatrist, or child’s paediatrician is required and must contain the information below. 

Note: A Mental Health Treatment Plan (MHTP) is not a referral. It is a treatment plan. We cannot submit Medicare claims on your behalf upon receiving a MHTP. Your doctor is required to provide a referral.

Referrals to psychologists remain valid until all treatment sessions requested/provided within the referral have been used. 

  • Referral Date
  • Patient’s Name, DOB, and address
  • Diagnosis
  • Request for Services under Better Access
  • The Number of Treatment Sessions Requested (maximum of 6 per referral)
  • Referring Practitioner’s Medicare Provider Number
  • Practitioner’s Signature
